Introduction: Primary ovarian non- Hodgkin lymphoma especially T-cell type is a very rare disease which is classified in the group of malignant small round cell tumors. In these tumors, for detection of primary tumor type, it is very crucial to perform more evaluation of the patient and disease, and also using of further pathologic cytologic methods such as immunohistochemistry, immunophenotyping by flow cytometry, RT-PCR (reverse transcriptase- polymerase chain reaction), FISH (flurescent in situ hybridization), electron microscopic approach and molecular genetics methods. This study was performed with the aim to demonstrate the importance of complement evaluations in patients with diagnosis of malignant small round cell tumor of ovary. Case report: In our report, a 31-years old woman with complaint of abdominal pain and diagnosis of ovarian mass has been evaluated. During evaluation process, she had several primary diagnosis, but eventually, with complement evaluation of immunohistochemistry, the final diagnosis of ovarian T-cell lymphoma was confirmed and based on it, treatment was performed. Conclusion: Malignant small round cell tumors in ovary need to several complement pathologic evaluations for confirmingofdefinitive diagnosis and planning of correct treatment.
